J.D. likes his patient's private practice doctor, Dr. Peter Fisher (Jay Mohr), but is shocked to find out he's the reason Jordan and Cox got divorced. Janitor tries to flirt with Elliot. Carla begins to doubt her sexiness.
The episode is told from Cox's viewpoint as he discusses the hospital and Jordan in his therapy sessions with Dr. Gross (Eric Bogosian). Elliot meets Paul Flowers (Rick Schroder), who she likes until she finds out he's a nurse.
Turk's brother, Kevin (D.L. Hughley) comes to visit with important news. Dr. Townshend (Dick van Dyke) runs into trouble with Kelso when J.D. screws up an out-dated procedure under Townshend's supervision.
Elliot: Dr. Cox! Um, Mr. Tillman is my patient, too, you know. Dr. Cox: He is? Elliot: Yeah, I was in the room with you this morning, remember? Dr. Cox: Uhhh, no. Elliot: Well, I was. Anyway, um, I-if you want, I could talk to his wife, because I... just really enjoy that kind of stuff. Dr. Cox: Wait a minute: Are you actually saying you enjoy telling people their spouses are going to die? Elliot: ...Yep! I mean, I know that-that sounds a little bit weird, but I-I-I-I just really like being there for people; it's... kind of why I became a doctor in the first place. Dr. Cox: Well, it's nice to know where the magic began
(J.D. winks at Dr. Cox) Dr. Cox: Oh my God, Sabrina. You had better tell me that you just had laser eye surgery and they accidentally severed the muscle that enables you to hold that lid up, because you did not just wink at me! J.D.: I didn't mean anything by it, I wink at everybody. (Winks at Dr. Kelso) Hey, Dr. Kelso! Dr. Kelso: Save it for the bathhouses, sport.
